At our bed and breakfast there were three bathrooms; two inside and one outside. (No hot water though). The family keeps a traditional outdoor bathroom because the water in Trinidad is always getting turned off. Unfortunately, it always seemed to be off when I wanted to take a shower.
Here's my favorite outdoor shower feature: the water window. The window connects the shower to the adjacent basin, which is full of water. You just lift the flap and fill your bucket.
